Office Policies

New Patients

We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, pre-register your demographic information and remind you to have a driver available to take you home after the appointment if needed. When you check-in for your appointment, we will need to copy your insurance card(s) in order to bill your insurance company.

Appointments/Cancelations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators. Your consideration in this matter will allow us to accomodate other patients who could benefit from an appointment.

Emergencies

We do not provide emergency coverage for patients. If you have an emergency, please call your primary care physician's office or 911.
